Financial/Insurance

FINANCIAL

We are committed to your child's treatment being a success.  Please understand payment of your bill is considered a part of your child's treatment.

  • Payment for treatment is due at the time services are rendered. We accept cash, checks and the following credit cards:  Visa, Mastercard, and Discover.
  • We are pleased to offer CareCredit. CareCredit is a dental financing option for patient offering 0% INTEREST and extended payment plans.  The business office will be happy to answer your questions regarding CareCredit and help you enroll.
  • The parent or guardian who brings the child for his/her visit is legally responsible for payment independent of what a divorce decree may state. We will not send statements to other persons.  Please make payments to the office in advance if someone other than the parent/guardian will be bringing your child to their appointment.

INSURANCE

As a courtesy to our patients, we are happy to file your dental claim for payment.  But please understand your insurance plan is a contract between you and your employer.  On each visit to the office you will be responsible for deductibles, co-payments, and/or balances not covered by insurance.

Every effort will be made to provide you with an accurate estimate of your cost for treatment, but please understand we are often limited by the information provided by your insurance company.  Before services are rendered, insurance only gives the office an ESTIMATE of your benefits and does not guarantee eligibility or payment.  Payment is only determeined after the claim is received by the insurance company for processing. Even if a procedure is medically and dentally necessary, it may be excluded from your dental plan.  This does not mean your child does not need the procedure.

  • It is imperative you provide all the necessary information to file dental claims on your behalf. This will include the insured’s personal information, a valid Dental (not Medical) insurance card with a phone number to verify benefits and a correct mailing address. If this information is not available at the time of the appointment or the insurance company can not confirm eligibility, you will be responsible for payment in full at the time services are rendered.
  • WE ARE NOT A CONTRACTED PROVIDER WITH ANY DENTAL PLAN; WE ARE CONSIDERED AN OUT-OF-NETWORK PROVIDER.
  • We are not responsible for any limitations in coverage that may be included in your plan.  If your dental plan denies a claim for any reason, you will be responsible for the bill in its entirety.
  • Patients with DELTA DENTAL INSURANCE please be advised: due to Delta Dental's refusal to accept “Assignment of Benefits” on most of their plans, payment in full is required at the time of service. We will continue to file all the necessary claim forms for your reimbursement.
